Job description
Summary
Sunrise Community, Inc. is a private not-for-profit organization dedicated to serving people with intellectual and developmental disabilities. The Program Coordinator supports individuals in living valued lives in the community by developing, implementing, and coordinating person-centered program plans and training programs, while ensuring accurate documentation and communication with support teams.
Responsibilities
- Responsible for the development, implementation and coordination of individual’s program plans
- Develop, implement and monitor individualized training programs to meet identified priority training needs
- Ensures all plans are person centered and reflect the individual’s hopes and dreams through active participation wherever possible
- Ensure all documentation is accurate and complete according to regulatory requirements
- Communicate with all team members, including family, guardians and support coordinators to ensure that the individual’s needs are being addressed
- Performs other duties as assigned
Skills
- Must possess a valid Driver's License with all necessary endorsements and have a minimum of 3 years of driving experience
- Must possess advanced computer skills
- Must possess a High School Diploma or equivalent
- Minimum one year of experience in writing program plans, case notes or other relevant documentation required
- Bachelor's Degree preferred
- Experience working in the field of intellectual/developmental disabilities preferred
Qualifications
Must Haves
