Job description

Summary

Community Health Network, in partnership with Deaconess Health System through the Innovative Healthcare Collaborative of Indiana, is seeking a Project Coordinator to support Population Health Operations initiatives. The role coordinates project plans, meetings, documentation, stakeholder updates, reporting, and process improvement efforts across the organization.

Responsibilities

  • Coordinate project plans, timelines, deliverables, and documentation for population health and process improvement initiatives
  • Facilitate project meetings, develop agendas, capture meeting notes, and track action items
  • Monitor project progress, risks, issues, and milestones while providing regular status updates to stakeholders
  • Coordinate and manage executive leadership calendars to support project update meetings and related activities
  • Collaborate with leaders, vendors, and cross-functional teams to support project success
  • Assist with reporting, presentations, metrics tracking, and project logistics
  • Support data-driven improvements that enhance population health results and operational effectiveness

Skills

  • Applicants for this position should be able to collaborate with others in a team setting, have excellent communication skills, and a positive attitude toward problem-solving
  • A bachelor's degree in a related field is required
  • Two (2) years of project coordination or related experience, preferably in healthcare or process improvement environments
  • Two (2) years of experience managing multiple priorities in a fast-paced setting
  • Strong organizational, communication, and stakeholder engagement skills
  • Experience with Microsoft Office applications, including Excel, PowerPoint, and Teams
  • Working knowledge of project management methodologies such as Agile, Scrum, Lean, or Waterfall
  • Ability to build relationships across all levels of the organization
  • Self-motivated, adaptable, and comfortable managing multiple projects simultaneously
  • Passionate about improving processes and supporting organizational initiatives
  • This position will allow flexibility to work remotely. Community caregivers performing work remotely are permitted to live in the following states: Indiana, Illinois, Ohio, Michigan, Kentucky, Florida and Texas. Caregivers are not allowed to perform work remotely outside of the above states. Applicants from other states may apply; however, if hired, they will be required to relocate to one of the above states within 60 days of their employment date
  • Project Management Professional (PMP), Lean Six Sigma, Scrum Master, Agile Certified Practitioner, or similar certification preferred
  • Healthcare, population health, or operational improvement experience preferred
